For all contracted and non-contracted providers, you may submit claims projects via e-mail using the spreadsheet located below. Once completed, please e-mail the spreadsheet to VBC-ProviderSupport@ConiferHealth.com
If you wish to file a grievance or appeal, this is not the appropriate medium to do so. All items containing information regarding appeals will be disregarded and forwarded to our compliance department. To file an appeal, please follow the plans appropriate process.

- All files sent must utilize the approved spreadsheet, any items not using this format will be sent back.
- All files must be e-mailed to VBC-ProviderSupport@ConiferHealth.com. Any faxed/mailed items will be not be considered and returned to the provider as they are unusable in their current state.
- Once received, you will receive an immediate automated response that the item was received. From this point, you will be contacted with 48 business hours regarding a ticket number and a point of contact.
